Medication Policy and Consent Forms

Parents must provide a consent form in order for students to receive medication during school hours. This includes over-the-counter (OTC) medications.

Parents must provide prescribed and over-the-counter medications to the school nurse. Please note:

  • Please leave the medication in the original container with the prescription label on it. Note that OTC medicines must also remain in the original container with the original label, with the student’s name on the container. The school nurse will not administer medications that are not in the original containers.

  • Students may not have any type of medicine (prescription or OTC) in their possession during the school day. Students must bring all medications to the nurse upon arrival, or a parent/guardian may deliver medication on the student's behalf.

Inhalers

If your student is required to use an inhaler, the Inhaler Self-Administration Form must be on file in the nurse’s office. The form must be completed by the physician as well as the parents/guardians. Students may carry their inhaler once the consent form is on file in the nurse’s office. 

EPI PENS

  • If your student is required to carry an EpiPen in case of emergency, the EpiPen Consent and Release Form must be on file in the nurse’s office. The  form requires completion by the physician as well as the parents/guardians. Students may carry their EpiPen once the consent form is on file in the nurse’s office.
